Information for your appointment
Dear patient,
We are pleased that you have decided to undergo treatment at the MCLINIC Interdisciplinary Medical Center Munich. In order to make your visit to our practice as pleasant as possible, we would like to offer you our support in advance and provide you with all the important information at a glance.
Checklist before the appointment
You can check whether you have gathered all the important documents for your visit to the practice by using our checklist. If possible and if available, please bring the following with you:
- Electronic health card (if you have statutory health insurance)
- Bonus booklet (if you have statutory health insurance)
- The referral and important preliminary findings from your referring physician (if available)
- Recent x-rays & x-ray passport (if available)
- Vaccination certificate, allergy passport and diabetic card (if available)
- A medication list, if you need to take medication on a regular basis
- The completed and signed medical history form (if available)
- The completed and signed consent form (if possible)
If you are a new patient making your first appointment with us, you will be asked to complete a questionnaire for your first appointment, known as a medical history form. In addition to your personal data, this contains information about your state of health. This is absolutely necessary for your and our safety. You can also state your wishes for the treatment. Of course, your information is subject to medical confidentiality. You must also complete and sign a declaration of consent for the transmission of treatment data and findings.
Forms for download
If you wish, you can download the medical history form and the consent form as a PDF file in advance at home, print them out, read them through at your leisure and bring them with you when you have completed them. This will also help us to shorten the registration time at the first appointment. If you have any questions, we will be happy to help you fill out the forms in the office.
Data protection notice
The protection of your personal data is important to us. According to the EU General Data Protection Regulation (DSGVO), we are obliged to inform you about the purpose for which our practice collects, stores or forwards data. The information also tells you what rights you have in terms of data protection.

If you have any further questions about your treatment at the MCLINIC Interdisciplinary Medical Center Munich, we will of course be happy to answer them. Please also let us know in good time if you are unable to keep your agreed appointment and have to postpone it.

OPENING HOURS
Our opening hours are Monday to Thursday from 08:00 - 12:00 or 14:00 - 18:00, and Fridays from 08:00 to 13:00. Appointments by reservation.
